These Terms of Service ("Terms") govern your access to and use of the PerMed Health membership, website at www.permed-health.com, and the PerMed Care mobile application (together, the "Services"), operated by PerMed Health ("PerMed," "we," "us," or "our").
By accessing our website, activating a membership, or using the PerMed Care app, you agree to be bound by these Terms. If you do not agree, please do not use the Services.
PerMed Health membership is extended by referral from an existing member and formal invitation from our team. Membership is limited and offered at our discretion. You must be at least 18 years old to hold a membership.
Access to the PerMed Care app is provided as part of an active membership and may be suspended if your membership lapses or is terminated.
PerMed Health provides a private preventive health membership that connects your personal health data, current clinical research, and dedicated physician access. The PerMed Care app allows members to view synced data from connected devices (such as WHOOP, Withings, and Apple Health), track recovery, sleep, and glucose metrics, and communicate with their care team.
Membership fees are billed according to the tier selected at enrollment (Individual or Couple+Family). Fees are billed on a recurring basis and are non-refundable except as required by law or as otherwise stated at enrollment.
You may manage or update billing details through the Member Portal. Membership billing is managed outside the PerMed Care app through the Member Portal or enrollment process; the app itself does not process membership purchases through in-app purchase. Failure to maintain valid payment may result in suspension of Services, including app access.
The PerMed Care app allows you to connect third-party health devices and accounts. By connecting a device, you authorize PerMed to receive and display the associated health data within your dashboard.
Information provided through the Services, including AI-assisted research summaries, is intended to support — not replace — the clinical judgment of your dedicated physician. Always consult your physician before making changes to your care based on information in the app or on our website.
All content, design, trademarks, and software associated with PerMed Health and the PerMed Care app are the property of PerMed Health or its licensors. You may not copy, modify, distribute, or reverse-engineer any part of the Services without our prior written consent.
To the fullest extent permitted by law, PerMed Health shall not be liable for any indirect, incidental, or consequential damages arising from your use of the Services, including reliance on data synced from third-party devices. Our total liability for any claim relating to the Services is limited to the amount you paid for membership in the twelve months preceding the claim.
PerMed may suspend or terminate your membership and app access for violation of these Terms, non-payment, or at our discretion with reasonable notice. You may cancel your membership at any time by contacting admin@permed-health.com.
These Terms are governed by the laws of the Commonwealth of Massachusetts, without regard to conflict of law principles. We may update these Terms periodically; material changes will be reflected in the "Last Updated" date above. Continued use of the Services after an update constitutes acceptance of the revised Terms.
